Instant packs, gel packs and cold spray are not the same item
An ice pack can be a chemical pouch, a frozen gel block or plain ice. Security reads each one differently.
- Instant ice packs sit in the medicines table
- Japan's main list names the instant cold pack in its medicines table, beside cold spray, taping spray and taping remover spray. The table is defined as covering medicines, quasi-drugs and things a person is expected to carry for treating injuries and for first aid. The TSA's entry, Instant Hot/Cold Packs, allows them in carry-on and checked bags with no special instructions.
- Plain ice and gel packs count as liquids
- The Japanese version of the liquids list counts ice among restricted items, in the row for ice cream and shaved ice, and gels fall under the rule by definition. A frozen gel pack in carry-on on an international flight is therefore read as a liquid item. Narita Airport allows a cooling agent for breast milk only if it is packaged with the milk, and asks for both to be declared to the inspector.
- Cold spray is filed with medicine, not with sports aerosols
- On international flights the liquids list keeps aerosols for sporting and daily use out of the cabin and permits them only in checked baggage, while toiletry and over-the-counter medication aerosols may go in the 100 ml bag. The main list places cold spray and taping spray on the medicine side. CASA words its own category as non-flammable aerosols for sporting, toiletry or medicinal use, intended to be applied to skin.

Do ice packs have to be frozen solid to go through airport security?
Only where the TSA's rule applies. The TSA lets freezer packs and gel packs through when they are frozen solid at screening; once partially melted or slushy they must meet the 3-1-1 liquids requirements. GOV.UK runs the other way: you cannot usually carry frozen items such as ice packs in hand luggage, and for ice packs carried with medicine its table says to contact the airport.
